NHC's registered nurses, therapists, and home health aides are here to help persons live comfortably in their home. They work with patients and families to identify key ways to maximize independence and learn to live comfortably in your home. NCH Homecare offers nursing services, including medication and health monitoring, patient education, wound and ostomy care, palliative care and pain management; physical, occupational and speech therapy. Home health aides offer care and supervision of routine daily activities during recovery from surgery or an illness, such as help getting dressed, bathroom safety, eating, personal hygiene, prescribed exercise programs and light housekeeping. NHC also offers social workers who can help identify local support programs and services, transfers to alternate levels of care, developing coping strategies for patients and their families, helping to find financial assistance resources and providing counseling for emotional difficulties.
Social workers can help provide constructive support to patients and their families. Working closely with nurses and therapists, they advise on a number of home health care issues, including:
Identifying local support programs and services
Transferring to alternate levels of care
Proposing coping strategies for patients and their families
Helping to find financial assistance resources
Providing counseling for emotional difficulties

Description
If your physician decides that home health care is right for you, after an injury or illness, NHC Home Care offers an experienced team of nurses and therapists who are dedicated to provide their patients with the knowledge, skills and care they need to recover, take charge of their own health and be able to remain safely at home. Most people who require these services are recovering from an illness, injury, surgery or are living with chronic medical conditions with acute changes requiring frequent changes in medication or other treatment. Nurses are available on-call to patients 24 hours a day, 7 days a week. Home Health Care Services, provided by Registered Nurses and Licensed Practical Nurses, include: Patient assessment Medication monitoring Intravenous infusions Wound care Ostomy care Catheter care Pain management Palliative care Patient Education Care giver education Physical Therapy, Occupational Therapy, Speech Therapy Home exercise program Therapeutic exercises Fall prevention programs Strengthening and balance exercises Post-surgical care to improve mobility Heart and Lung deficit exercises
