Description:

Helps individuals with functional limitations stay in their own home by providing several in-home services; light housekeeping, assistance with bathing, home delivered meals, and case management. To be eligible, individuals must be physically unable to do at least 3 activities of daily living for themselves, such as housekeeping, shopping, bathing, walking without assistance, etc. There is no income requirement unless their income is over the monthly amount of the Federal Guideline, and then the individual will have to pay a cost share.

Mon - Fri 8:00 am - 4:30 pm

Intake Process:

For information and to apply, the individual or representative must call the Intake Line at 1-866-836-6678. The Intake Specialist will determine eligibility.

Information needed to apply: name, date of birth, phone, address, marital status, health information including any health conditions, hospitalizations within the last year, any falls in the last 3 months, and any physical limitations. A copy of the intake form can be downloaded from the website: https://www.ethra.org/programs/11/options-for-community-living/.

Program Fees:
