Description:

This program helps individuals age 60 and older, and adults with disabilities who have functional limitations to stay in their own home by providing several in-home services: light housekeeping, assistance with bathing, home delivered meals, and case management.

Mon - Fri 8:00 am - 4:30 pm

Intake Process:

To apply, individuals or their representative can call the tollfree Intake Line at (866) 836-6678. An Intake Specialist will gather information and will determine eligibility. Information needed to apply: date of birth, marital status, household income, health information including any health conditions, hospitalizations within the last year, any falls within the last 3 months, and any physical limitations. Program Fees:

No cost to low income individuals. If the applicant's income is over the monthly amount of the Federal Poverty Guidelines, the applicant will pay a cost share.

Eligibility:

Individuals who are 60 years or older, or adults with disabilities who have functional limitations. Applicants must be physically unable to do at least 3 activities of daily living for themselves, such as housekeeping, shopping, bathing, and walking without assistance.
