Description:

Primary care clinic that provides primary and specialty health care to people who are uninsured or under-insured. Does not provide dental, oncology, prenatal care, or mental health services. Provides comprehensive care through a network of community partners including physicians, diagnostic centers, hospitals, and labs.

Intake Process:

Call to make an appointment. Appointment required. Walk-ins will not be accepted.

All new patients must bring immunization records.

If applying for sliding scale fee, patients must bring proof of household income. The following documents are acceptable proof:
* Last three paycheck stubs
* Last year!€™s tax returns
* Food stamp letter
* Notarized letter from employer on letterhead with his/her signature, phone number, and your monthly income
* Letter from friend/family stating their financial support for you

Program Fees:

Fee vary. Sliding Scale fee is available for patients who are unable to pay full amount.

* Clinic accepts HCC health insurance for refugees. No other forms of insurance are accepted.

Languages:

Arabic and Spanish available on-site. Translation services for other languages are available via Language Line.

Volunteer Opportunities:
