Provides services to children with developmental delays in Sumner County. The program benefits children from birth up to 4 years who qualify with physical or mental delays or by diagnosis. Home-based services are offered.

Mon - Fri 8:00 am - 4:00 pm
Intake Process:
Call for information and intake. Requires referral from Tennessee Early Intervention System(TEIS); TEIS can be contacted by calling 1-800-852-7157 or 931-528-1623
Program Fees:
No fee for children who are eligible
Eligibility:
Children from birth to 48 months of Age in Sumner County who qualify; children must have developmental delays (physical or mental delays) or by diagnosis. Children's eligibility is based on a screening and developmental test given by the Tennessee Early Intervention System (TEIS).
