Southland Mall Public Health Clinic - Immunizations/Well Child Checkups

Description:

Provides adult, adolescent and children's immunizations, flu shots and travel immunizations. Travel immunizations currently available at Hickory Hill, Millington, Shelby Crossing and Southland Mall clinics. Provides childhood immunizations for diphtheria/pertussis (whooping cough), tetanus, meningitis, hepatitis A and B, polio, measles, mumps, rubella; adult and international travel shots and typhoid. (NOTE: Only the main Immunization clinic on Jefferson offers the rabies and yellow fever vaccine). Offers HPV vaccinations for pre-teen and teen females.

This clinic also provides Well child checkups including physical exams and screening tests for individuals from birth to 21 years of age that are required for school entry. Referrals for medical and dental care are made when needed. Children receiving TennCare benefits get these preventative services on a specific schedule based on the child's age.

Program Fees:

Call for details on fees (fluctuates with the season and sometimes late in the season flu shots may be free); $13.00/single shots; No child denied immunizations, if unable to pay. Flu shots: Adults: $35.00/seasonal flu shot; $25.00/Children's flu shots (free shots to those covered by Medicare and TennCare); Well child checkups are $85.00 without insurance; free to children on TennCare

Eligibility:

Any resident of Shelby County. Immunization records available at any health department clinic. Records accessible by patient name, birth date, or mother's name.
