Description:
Offers Case Management services for individuals experiencing homelessness and are seeking permanent housing. Applicants must have had the CHAMP (Coordinated Housing Assessment Match Plan) assessment from The Bush Family Refuge in order to receive services. Once in housing, a case manager supports client while in housing to ensure success.
Hours:
Mon thru Thu 7:00 am - 5:00 pm
Fri 7:00 am - 4:00 pm
Intake Process:
Must have a CHAMP referral from the Bush Family Refuge
