Program Fees:
Center membership fees are $180 annually. Members of the Bellevue YMCA, who are 50 and older, will receive J.L. Turner membership with their YMCA membership. Scholarships for center membership, only, may be available. Fees for classes or activities may vary.

*Must be a member of the Bellevue YMCA to access YMCA equipment and services.

Description
FiftyForward Care Team is a program that provides care management specifically for older adults, ages 50 and older, who have a limited income and don't already have access to a care manager. Benefits: *In-home visit to determine needs *Linkage, application, and advocacy with service providers *Monitoring to ensure that services are received in a timely manner

Conservator Program is for individuals who can no longer make safe or appropriate decisions for themselves due to cognitive and/or physical disabilities. Benefits include: *A healthcare advocate for decision making *Assistance in managing income and assets *Help in maintaining an active lifestyle, family, relationships, and independence. *Regular in-home visits *Court oversight of all decisions made regarding sale of property and management of finances *24/7 access to staff

Delivers meals to adults, aged 50 and older, who have difficulty preparing their own meals or lack the resources to plan and prepare meals and live in Davidson County, TN. Special dietary restrictions can usually be accommodated, and customers will be given a non-perishable meal on the days that the program is closed for holidays.
