FiftyForward Care Team is a program that provides care management specifically for older adults, ages 50 and older, who have a limited income and don't already have access to a care manager. Benefits and services include: - In-home visit to determine needs - Linkage, application, and advocacy with service providers - Monitoring to ensure that services are received in a timely manner

Conservator program for individuals who can no longer make safe or appropriate decisions for themselves due to cognitive and/or physical disabilities. Victory Over Crime is a program for older adults, who have been victimized by crime in Davidson County. Services include, but are not limited to case management, legal advocacy, and counseling support.

Description
FiftyForward (Meals on Wheels) delivers meals to adults, aged 50 and older, who have difficulty preparing their own meals or lack the resources to plan and prepare meals and live in Davidson County, TN. Special dietary restrictions can usually be accommodated, and customers will be given a non-perishable meal on the days that the program is closed for holidays.
