Description:
Provides a variety of programs to assist adults 18 years old and over with mental illness and co-occurring disorders of mental illness and substance use. Park Center, Main office location offers co-occurring services (Intensive outpatient and outpatient treatment programs), IPS (Individual Placement and Support Method) supported employment, and residential services including permanent supportive housing or a residential treatment program.

Intake Process:
Go to website (www.parkcenternashville.org) and complete the online referral form and the release of information form (ROI). The Intake Coordinator will follow up within 48 business hours. To apply for the housing or supported employment program, client will need to provide one month's worth of pay stubs or the most recent award level from SSI/SSDI and other needed documents.
Program Fees:
For residential treatment program, must have TennCare and be able to provide rent payment of $500 - $625, depending on the unit location (includes food and utilities); the permanent supportive housing program requires applicants to have some form of income and meet other eligibility criteria; Outpatient program is free for persons on TennCare; Inpatient program is free for those on TennCare and may be free for eligible uninsured persons through the TDMHS grant; supported employment program is free

Eligibility:
For Permanent supportive housing program: Adults with a diagnosis of serious mental illness who are residents of Davidson County.
- Must be sober for at least six months (or three if in treatment)
- Must have income and be able to pay rent (either 30% of income or 375 depending on the unit) and daily living expense.
- Cannot be on the sex offender registry or have an intellectual disability.
For Residential Outpatient Treatment program: Adults (ages 18 and over) with a diagnosis of mental illness and current/recent substance abuse.
- Priority admission is given to women who are pregnant and people who are using substances through IV injection.
- Not appropriate for persons with intellectual disabilities who have an IQ less than 70.
- Must meet criteria for the requested level of service (IOP or OP), based on the American Society of Addiction Medicine.
Supported Employment program serves persons in Davidson, Rutherford, Sumner and Wilson Counties

Description
Provides two different outpatient programs for adults with mental illness and substance use disorders: 1. Intensive Outpatient Program (IOP): All day individual counseling, group counseling, and educational groups, random drug screens, and employment services. 2. Outpatient Program: 9 hours a week for a more flexible schedule. provides same services as IOP.

Description
Program offers affordable, permanent, supportive housing for people living with serious mental illness and co-occurring disorders. Units are located throughout Nashville and are offered to single, unaccompanied adults (no partners or kids). Program is for people interested in a sober lifestyle, and staff support is focused on increasing stability, accessing resources and employment, and increasing community involvement.

Description
Supportive Housing is the second kind of housing program offered by Park Center. This program provides psychiatric rehabilitation (PSR) services to help adults who have serious mental illness to learn the skills needed to transition to independent living. Staff provides individualized coaching and educational groups on wellness, cooking, cleaning, and hygiene. Staff is on duty 24/7.

- For adults living with a serious mental illness or co-occurring disorder. - Separate men's and women's houses, with up to 8 residents each (2 people to a room): 24/7 on -site staff support. - Requires TennCare & rent payment of $500 (includes food and utilities) - No time limit to services, though TennCare must continue to authorize consumer's eligibility. - Unable to house people on the sex offender registry.

Description
Provides assistance to individuals obtaining employment through planning and developing goals. Supported services are provided to ensure job retention, including transportation assistance and job coaching. Program utilizes the IPS (Individual Placement and Support) method.
