Provides case management, financial assistance (limited amount & zip code requirements), food boxes (limited to once a month), infant and small child items (diapers, baby formula, clothing and supplies), hygiene items, household items and cleaning supplies, MDHA housing registration assistance, SNAP enrollment assistance, and referral/linkage to other agencies and non-profits.
Persons in need of counseling, individual and family, are referred to the main office (615-352-3087) in Donelson at 2806 McGavock Pike.
The financial assistance program offers very limited amounts of utility (electric, water and gas) and rent or mortgage assistance to residents of the following zip codes/areas: 37143 (the Nashville-Davidson County side only); 37189; 37203 (northern portion only, near Charlotte Avenue); 37205; 37208; 37209; 37218; 37221; and 37228. Financial assistance for rent, mortgage, electricity, water, or gas help is zip code specific.

Intake Process:
Intakes are processed Monday through Friday 8:30 am-5:00pm. Walk-ins and appointments are accepted. Intake will be completed by the Intake Specialist. Clients are forwarded to the Case Manager for Case Management Services and Financial Assistance. All Clients should have a valid ID. Clients seeking rent or mortgage assistance should bring a current lease, a recent mortgage statement or rental ledger.
Food pantry or other types of assistance only require a photo ID showing residence in Davidson County and/or age; the commodities program requires more documentation; call for details.

Description
The CSFP (Commodity Supplemental Food Program) is open to persons 60 and older with low incomes in Davidson County. Once registered, seniors can pick up food according to distribution schedule.
