Description:

Tennessee's local health departments provide a variety of health services including vaccines, family planning, STD control, WIC, health education, and more. This site also offers limited dental services. This site offers well child checkups which include physical exams and screening test. Screenings are available to assure normal development and to detect and treat many medical conditions for people from birth - 21 years of age. Can provide copies of birth certificates for persons born on or later than 1949 in TN and copies of death certificates which are issued upon request, regarding deaths in Tennessee. Provides family planning for patients with a complete physical exam and appropriate lab tests, including pap smears. Patients are taught about birth control and given birth control. Referrals for vasectomies are provided for men. Provides all CDC recommended childhood/adult vaccines: including polio, whooping cough, tetanus, measles, mumps, rubella, flu, pneumonia, HPV, etc. Provides clients with pregnancy testing, presumptive eligibility screening for Medicaid, and referrals for prenatal care. Offers the WIC Program, which provides food benefits to buy nutritious foods (cheese, milk, cereals, etc.), nutrition education, breastfeeding support, and more to women who are pregnant or breastfeeding and children under five years of age who are at risk of poor growth. Also offers the CHANT program, (Community, Health, Access, and Navigation in TN), a free, voluntary care coordination service that assists families in identifying resources in their community to meet medical or social needs for Pregnant/postpartum, children ages 0-21 years, and children with special healthcare needs; and GIFTS (Growing Inside Free of Tobacco and Smoking), an incentive-based smoking cessation counseling program for pregnant individuals and their partners, which offers up to $300 in rewards for program completion. Also offers Breast and Cervical Cancer Screenings (BCS Program), which helps low-income, uninsured women gain access to timely breast and cervical cancer screening and diagnostic services. This service is free for Tennessee residents who meet age and clinical requirements for breast cancer screening (ages 40+), with income at or below 250% of the Federal Poverty Level for family size and who are uninsured. Primary care services are provided for persons with no health insurance ages 19-64 by licensed medical staff at many health departments, including Bedford, Lawrence, and Maury County health departments. Services include physical exams, basic health care, treatment of acute illnesses, advice on preventative behaviors and healthy lifestyles, management of common chronic diseases, Telehealth (connected with provider virtually), diagnostics, basic lab services, treatment, referral, and care coordination.

Fees vary by service; sliding scale fee for most programs based on family size & income (also accepts TennCare and private insurance); some programs (Breast & Cervical Cancer Screening, WIC, CHANT & GIFTS program) are free if income and other eligibility criteria is met. Primary Care Services: Cost based on family size and income using a sliding fee scale with a minimum fee of $10. There may be additional charges, based on the sliding fee scale, for office visits, lab work, medications, immunizations, or other procedures.
