Eligibility
Disabled driver or passenger license plates, placards and decals are available to Tennessee residents who: are confined to a wheelchair. walk with difficulty or uncertainty. have 20/200 vision or worse with corrective lenses. are the parent or legal guardian of a person who is permanently disabled and incapable of operating a motor vehicle. An agency can obtain a disabled placard if it: transports disabled clients; uses the placard only when transporting disabled clients; and secures just one placard per vehicle owned and operated for the purpose of transporting disabled clients. Temporary disabled placards are: available to those with a temporary disabling condition. valid for up to six months. reissued once, if necessary. A disabled driver decal is available to people with a permanent disability who currently own or purchase a specialty plate.
Fees
Disabled plate â $26.50 annually (no charge if confined to a wheelchair). Permanent disability placard â $26.50 + $3 renewal every two years (no charge if vehicle registration in applicantâs name). Temporary disability placard â $10. Replacement placard â $2. Disabled driver decal â no charge.
Intake Procedure
To apply for a disabled person license plate/placard/decal: 1. Complete the "Application for Disabled Person License Plate, Placard and/or Decal." Applications are also available from your county clerk. 2. Provide certification of disability from either: a medical doctor licensed to practice medicine; a physician's assistant or nurse practitioner acting in conjunction with a written protocol developed jointly by a physician; or a Christian Science Practitioner listed in the Christian Science Journal 3. Submit the two forms to your local county clerk.

To title and register a motor vehicle in Davidson County, the owner must possess a State of Tennessee issued Driver License/I.D. , or provide two documents that prove residency in the county. In the case of a commercial vehicle, physical presence must be established, such as the owner's principal place of business or an office or place of business maintained by the owner. While a resident or commercial business owner may use a mail service provider like a post office box, to receive incoming mail, they may not use this address as the sole basis for establishing a bona fide residence or physical presence. Acceptable proofs of residency and physical presence include the following: Current utility bill including the postmarked envelope it was mailed in Current bank statement Current rental/mortgage contract fully signed and executed or receipt including deed of sale for property Current TN voter registration For used vehicles: When purchasing a used vehicle from an individual you should make sure the seller properly reassigns the Certificate of Title. The seller should sign under the Assignment of Title by Registered Owner heading. MAKE SURE THAT THE PERSON'S NAME YOU ARE PURCHASING THE VEHICLE FROM APPEARS ON THE FRONT OF THE TITLE AND THEY HAVE SIGNED EXACTLY AS THEIR NAME(S) APPEARS ON THE FRONT OF THE TITLE. Please note that only a licensed dealer may pass ownership without titling the vehicle in their names. If there are two names on the front of the title and the names are joined by the conjunction AND, both parties must sign the back of the title. Upon receipt of the Certificate of Title you should sign to accept ownership of the vehicle. You must have a valid plate to transfer to the purchased vehicle, or you will need to purchase a new plate. In addition, all new and renewal registrations require an emissions certificate. Individuals new to Tennessee will also need to provide the following: the most current out-of-state registration, and the name and address of the lien holder to whom you are making payments. If you do not have a lien holder, you must furnish your out of state title.
