Description:

Authorizes legal name changes for any Shelby County resident. Also provides legal authorization to make changes to errors in birth certificates.

Hours:

Court Hours: Thu 1:00 - 2:00 pm

Intake Process:

Call or visit website for information; some forms can be downloaded from the website or picked up form from the office. You do not have to have an attorney to make these changes or corrections, but you must have the Petition and Order forms ready to be presented to the judge; forms are available from the website.

Program Fees:

$166.50/name change or birth record correction; certified copy of Elvis Presley's will/$11.50; accepts cash, credit and debit cards (MasterCard or Visa)

Eligibility:

Residents of Shelby County who are 18 and over can change their name without parental consent. Must provide birth certificate and proof of identity and residency. NOTE: If you are changing the name of a minor child, both parents must be present OR have a notarized statement from the absent parent.
