Description:

Provides a network of community agencies which provide home visits and site-based services that promote good health for pregnant women and their families in Shelby County through the delivery of coordinated services for children prenatal through age eight and their families. Special emphasis is placed on services that prepare children to be ready for school. Coalition is now housed at Porter-Leath. DTD Partner Agency. Translator services available.

Mon - Fri 8:00 am - 5:00 pm

Intake Process:

Call for information or fill out the one page Network Client Referral Form on the website. Referrals accepted from clients, physicians, nurses, family and friends. Each agency has specific enrollment requirements and targeted age groups; but service providers can refer a client to another agency in the ESCN when appropriate.

ParentPlus is a website with an online portal that guides families to available programs. All programs are voluntary and free of charge to eligible families in the county; go to website for information: www.parentplus901.org

Program Fees:

No Fee for home-visiting services for eligible teens, women and their families; Site-based services that include pre- and postnatal medical follow-up are covered by TennCare; TennCare is not required for the home visitation services. However, if the client is not enrolled in TennCare, the agency program staff will help the client enroll.
