Description:

Early intervention services for children with disabilities assist parents in maximizing their child's abilities: services include consultation by various professionals and home visits. This office serves Benton, Carroll, Crockett, Dyer, Gibson, Henry, Houston, Humphreys, Lake, Lauderdale, Obion, Stewart and Weakley counties. This program is supervised by the TN Department of Intellectual and Developmental Disabilities (TN DIDD).

Hours:

Mon - Fri 8:00 am - 4:30 pm

Intake Process:

Call to schedule an appointment or be referred; may also fill out online referral form.
Documents Needed: Birth Certificate

Program Fees:

Evaluation and service coordination at no cost to eligible children. Accepts Medicaid/TennCare, Private Insurance

Eligibility:

Children can be screened for eligibility via TEIS office in their vicinity. Referrals are also received by physician, hospitals, DHS, etc. or others
