Description:
Provides rent/mortgage assistance, utility assistance, summer camp for children, gas vouchers, and burial assistance to individuals living with sickle cell.
NOTE: Due to COVID-19, The Sickle Cell Foundation of TN is offering a food voucher to individuals living with sickle cell disease. Individuals must live in Davidson or Robertson County. Apply by phone, Mon - Wed from 9:00 am - 4:00 pm at 901.552.4267 or at www.sicklecelltn.org. Proof of illness must be provided by a medical provider.

Provides up to $100 in utility payment assistance to sickle cell patients in need. A maximum of $100 per calendar year may be paid based on available funding. Funding is first-come, first-served. These funds shall be used as the payer of last resort; therefore, clients must exhaust other community resources prior to receiving utility assistance through SCFT.

Eligibility
Adults 18 or older living with sickle cell. If a client has Medicaid, he/she is not eligible to receive transportation services through SCFT unless all of their transportation benefits have been exhausted. In order to receive transportation to a medical appointment, the client must contact their Medicaid agent to determine transportation policy.

Intake Procedure
If the client lives outside of the Shelby county area, he/she must contact a member of The Sickle Cell Foundation of Tennessee (SCFT) staff (901.552.4267) at least 72 hours before the scheduled appointment to request transportation. If the client lives within Shelby County he/she must contact a member of the SCFT staff at least 8 hours prior to the scheduled appointment to request transportation. If a client has access to a personal vehicle to make primary Sickle Cell Disease (SCD) medical, dental, or mental health appointments, a gas card* will be issued as reimbursement to supplement gas expenses. In order to receive a gas card, the client must provide evidence prior to receiving a gas card confirming that he/she has an appointment scheduled.

Description
Each summer, the agency hosts the Sickle Cell Kids Camp at The Center of Courageous Kids Camp in Scottsville, Kentucky. The camp is special because it is designated exclusively for children with serious and/or life-threatening illnesses. At summer camp, the campers are surrounded by energetic and caring counselors that facilitate creative and fun activities throughout the week. During the week there are qualified medical professionals on-site to offer assistance and provide support when needed.

All children age 7-16 living with sickle cell disease are eligible to apply. Campers associated with the Sickle Cell Foundation of Tennessee will be given priority.
