Mon - Fri 8:00 am - 4:30 pm (After hours has answering service)

Call the Eligibility Office (901) 523-8990 x 7972 or walk to Room CW130 at VAMC, 1030 Jefferson, Memphis, TN 38104 to apply for services.
Program Fees:
VA Memphis handles billing for this clinic
Eligibility:
US veterans enrolled through VA Medical Center in Memphis (must enroll in Memphis VAMC to use this clinic)
