8:00 AM- 5:00 PM; closed for lunch from Noon-1 PM

Intake Process:

Must complete application and provide all required documentation and be screened prior to receiving an appointment. Applications are available on our website or at our clinic.

Program Fees:

Vision Patient Visit - $20
Eyeglasses - $25
Chiropractic Patient Visit - $15

Eligibility:

Must reside in Abingdon VA, Bristol VA, Bristol TN, Blountville TN, Bluff City TN, Elizabethton TN, or Piney Flats TN.

Patient and/or someone in their household MUST be employed, unless they are retired or a student.

Patient must have NO Private Medical, TennCare/Medicaid or Medicare Insurance.

Total household income may not exceed 250% of the federal poverty guidelines:

Family of 1 person: Household income $2,831
Family of 2 people: Household income $3,815
Family of 3 people: Household income $4,798
Family of 4 people: Household income $5,781
Family of 5 people: Household income $6,765
Family of 6 people: Household income $7,748
