For seniors looking for legal services, GNRC contracts with the Legal Aid Society of Middle TN and The Cumberlands (Legal Aid). The Greater Nashville Regional Council (GNRC) helps to promote, screen, and coordinate a variety of home and community-based services on behalf of state and federal funding programs. The programs focus on connecting applicants to agencies that provide a range of life-sustaining support services, including personal care visits and homemaker services. The TennCare CHOICES Program provides long-term care for clients who qualify for Medicaid and meets the level of care that could be provided in a nursing home. Personal care visits and homemaker services are some of the many services provided through TennCare CHOICES. OPTIONS for Community Living is a state-funded program that, like CHOICES, offers personal care and homemaker services. OPTIONS is considered a last-resort program for those who do not qualify for CHOICES. The first step to determining which program is right for an individualâs needs is to call the Information and Assistance hotline. During the initial call, a member of GNRCâs Information and Assistance team will ask questions to learn more about the specific situation and needs. Depending on the type of service needed, an in-person assessment may be provided.
Hours
Mon - Fri 8:00 am - 4:30 pm
Eligibility
To qualify for CHOICES, you must be a Tennessee resident who is 65 years of age or older, or 21 years of age or older with a physical disability and need the level of care provided in a nursing home and qualify for Medicaid long-term care. To be eligible for OPTIONS, the applicant must be a Tennessee resident who has difficulty caring for themselves or their home. If you are determined eligible for CHOICES or other programs, the OPTIONS program cannot provide services.

Description
The Greater Nashville Regional Council (GNRC) provides free Medicare counseling and acts as the single point of entry for TennCare Medicaidâs long term care program called CHOICES. GNRC is part of a national network of State Health Insurance Assistance Programs (SHIP). SHIP is a Federal Administration for Community Living (ACL) grant program that helps states enhance and support Medicare beneficiaries through local programs, staff, and volunteer counselors. GNRCâs Medicare Counselors provide free, confidential, and non-biased information to Medicare beneficiaries, their representatives and those who will soon be eligible for Medicare. GNRC helps to promote, screen, and coordinate a variety of home and community-based services on behalf of TennCare CHOICES. This program provides long-term care for clients who qualify for Medicaid and meets the level of care that could be provided in a nursing home. These programs are intended to empower seniors, disabled adults, and other targeted populations to remain independent in their home and/or community for as long as possible.

Description
TennCare CHOICES in Long-Term Care is TennCare's program for long-term care services. Long-term care includes help doing everyday activities that you may not be able to do for yourself as you grow older, or if you have a physical disability. These are things like bathing, dressing, and preparing meals. Long-term care can be provided in a nursing home. Long-term care can also be provided at home, in the community, or in residential places like Assisted Living Facilities. Services to help a person stay at home or in the community are called Home and Community-Based Services (HCBS). The care you receive in CHOICES depends on your needs. CHOICES Home and Community Based Services (HCBS) may cover personal care visits, attendant care, homemaker services, home delivered meals, personal emergency response systems, adult day care, in home respite care, inpatient respite care, assistive technology, minor home modifications, pest control, community-based residential alternative and nursing home care.
