Description:

Provides free primary medical care and mental health care for working uninsured residents of Coffee, Franklin and Moore Counties. The agency now also accepts patients in these counties who are applying for disability, receiving disability without insurance, full time students, and those in transition between jobs (6-month allowance). Services include well woman and well man exams, check ups, sick visits and chronic disease management, specialty care referrals, telehealth visits and lab services. Mental Health services are on site only and include medication management by a psychiatric nurse practitioner and mental health counseling facilitated by a licensed counselor. Wednesday appointments are now held at a different site at 1914 McArthur Street, Manchester, TN 37355

Hours:

Mon - Wed 8:00 am - 1:00 pm; Thu 8:00 am - 4:00 pm (closed 12:00 noon - 1:00 pm); Note: In addition to regular hours, the clinic is also open every 2nd Saturday from 9:00 am - 1:00 pm. Wed clinics are now held at the new Manchester site at 1914 McArthur Street, Manchester, TN 37355

Eligibility:

Uninsured and working residents of Coffee, Franklin and Moore counties whose household income is at or below 250% of the Federal Poverty level; agency now also accepts patients in these counties who are applying for disability, receiving disability without insurance, full time students, and those in transition between jobs (6-month allowance).
