Description
Provides adolescents and adults with sever emotional disturbances plan, access and coordinate mental health and supportive services.
Hours
Hours vary depending on client and case manager. Services last anywhere between 3 and 4 months.

Eligibility
-Individuals who are transitioning from a higher level of care or do not meet medical necessity criteria for a more intensive service. -Individuals may also receive Case Management services in conjunction with Intensive Outpatient and/or Medication Management to coordinate care and engage client's external support systems.

Description
Provides integrated treatment for co-occurring substance abuse and mental health issues to both youth (12-17) and adults 18 and older. Services are designed to prevent higher levels of care, help sustain long-term sobriety, good mental health and support clients remaining in their permanent homes and environment.
Hours
Hours vary per clients and program, but groups meet up to 4 times per week and up to 9 hours per week.

Eligibility
Eligibility determined after assessment by designated staff. It is designed for adolescents 12-17 and adults 18 & older who will benefit from substance abuse and/or behavioral health treatment in an intensive environment but whose condition does not warrant detoxification or more intensive 24-hour monitoring.

Funded through: Medicaid/TennCare and Private funding.
Intake Procedure
Call the Nashville Region office for more information and a brief screening. After a screening a counselor will schedule an evaluation, if the individual qualifies for IOP they will be contacted by a counselor as soon as possible.

Description
Provides medication management to other program and service recipients. Provides quality and timely community-based assessment and medication management to the children and families we serve; and to expand via tele-med to serve rural areas of greatest need.

Service recipients who may be referred to Health Connect Clinic are: service recipients involved with in-home, outpatient, intensive outpatient, case management/health link services as well as immediate family members (siblings/parents) of service recipients served through in-home therapy programs.

Description
Provides mental health outpatient services that offer psychiatric and/or mental health services to children and adults who require treatment. Service provides opportunities for individuals to learn about their mental health and/or substance abuse issues, and how to better address those issues themselves.

Children and Adults that present with an array of mental health conditions and show signs of one of the following criteria to receive outpatient services: exhibits deficit in participating in social systems or activities, is at risk of developing maladaptive coping strategies, presents an increased risk of personal stress, or if the individual has co-occurring mental health and substance abuse disorders.
