Provides comprehensive medical care, dental care, and behavioral health services including pediatrics, women's services, physical examinations, immunizations, family planning, lab services, pharmacy, medical imaging, nutrition assistance and diagnosis and treatment of common ailments. Services are provided on an ability-to-pay basis and target low-income and indigent residents of the community.

Hours:

Clinic hours: Mon - Fri 7:30 am - 5:00 pm; Sat 8:30 am - 1:00 pm

Intake Process:

Walk-in or call for an appointment. Documents needed: a photo ID such as a driver's license, work I.D., etc.; a list of the medications and supplements such as vitamins you take; your medical history (any surgeries or serious illnesses you have had, your family health history, immunization records, etc.); your health insurance card if you are insured; if you aren!€™t insured and want to apply for discounted fees, please bring proof of your income. This can be your W2, most recent pay stub, most recent tax return, bank statement, a notarized letter from your employer or from an assisting agency who knows your financial status or a Social Security letter.

Program Fees:

TennCare, Medicare, CoverKids, sliding scale, most private insurance. Medical visits start at $20 and dental visits start at $30.
