Project Access - Johnson City

Description:

Care coordination hub for the Appalachian Highlands Care Network. Offers referrals to low or no cost Primary care, specialty care, hospital inpatient, outpatient, and ancillary services, and some discounted pharmacy and medical equipment services. We connect the uninsured to specialty medical care services.

NOTICE: Due to COVID-19, appointments are mostly by phone. Patients can drop off documents at front desk or in drop off box located outside of the suite.

Hours:

Monday-Friday, 9:00am-5:00pm

Intake Process:

Must be referred by physician or community health clinic to receive specialist appointments, but can be self-referred or referred by community resources for assistance in connecting to other services.

Eligibility:

Must have income 225% below federal poverty level, not eligible for health insurance or other benefits and live in our 21 county service area.
