Smile on 65+ Senior Dental Program - West TN

Description:

Smile on 65+ program provides low cost dental care for seniors (65 or older) who are residents of west TN who are dentally uninsured or underinsured (this means that if you don't have comprehensive dental insurance you MIGHT be eligible for the program) and meet income requirements. Services can include dentures, partial dentures, cleanings, emergency care, preventative services, x-rays, extractions and fillings. Some clinics may be able to do crowns or root canals on a sliding scale fee. Customers in need of dental care should call the customer hotline at 1-800-509-6055 for pre-screening and to find out if they are eligible. Services will be provided for west TN residents through seven regional clinics, (Church Health Dental Clinic at the Memphis Crosstown Concourse, Christ Community Dental Clinics in Memphis (Frayser, Third St, Broad St, Austin Peay and Hickory Hill) and Reelfoot Rural Ministries Dental Clinic in Obion, TN) but all clients must call the hotline to be pre-screened and cannot go to or call the clinic directly.

Sponsored by Interfaith Dental Clinic, which is based out of Nashville, with a mission to create a healthier community by providing oral health care for those experiencing poverty.

Reelfoot Rural Ministries will provide services for Obion County and Surrounding Areas.
Church Health Dental Clinic and five Christ Community Health Services dental clinics will serve Shelby County and Surrounding areas.

Hours:

Monday - Friday 8:00 am - 4:00 pm

Intake Process:

Clients are required to Call to be pre-qualified and for eligibility information (Walk-ins not accepted); ask hotline staff for referral to preferred available clinics. Client should bring a state ID, health insurance card and proof of income to their appointment.

Program Fees:

$25.00/office visit once applicant is qualified; some sites may offer work on crowns and root canals for a sliding scale fee (this would be an additional fee to the $25.00/office visit)

Eligibility:

Applicants must be at least 65 years old, be dentally uninsured (have no comprehensive dental insurance; call for specific eligibility requirements), be a TN resident, and be at 200% or lower of the Federal Poverty Income Level
