NOTICE: KDC (Keystone Dental Care) provides basic dental services to UNINSURED residents of NE Tennessee who do NOT have TennCare, are US Citizens aged 19 and older, and are under 200% of poverty guidelines.

Provides basic dental services, such as:
Exams
Cleanings
X-Rays
Fillings
Extractions
Partials
Dentures

Hours:

Monday-Thursday; 8:00AM-Noon, 1:00PM-4:30PM
Closed: Friday, Saturday, Sunday.

Intake Process:

Step 1: Complete online application (www.keystonedentaljc.com) or paper application (available at office) along with ALL required documentation.
Step 2: Application will be screened and verified.
Step 3: Applicant will be contacted via phone for an appointment time.

Program Fees:

Sliding scale fee (except for dentures), which is collected at time of service.

Eligibility:

MUST:
Be UNINSURED and not have TennCare.
Be a resident of Northeast Tennessee.
Be 19 or older.
Be a U.S. citizen.
Have valid photo ID and social security card.
Provide proof of income and be under 200% of poverty guidelines.
