Description:

Provides EBT card for purchase of food items only; expedites emergency services (receipt of card within 5 days after being determined eligible) for families/individuals with little or no income. Persons without an address may use that of a shelter, friend, or Dept of Human Services. Customers may walk in to their local DHS office to apply, apply online or ask for an application to be mailed to you; call the Family Assistance Service Center tollfree number (866-311-4287) to request changes to existing food stamp case or to request that an application be mailed to you; along with the application you will receive a pre-paid envelope to return the application to the local office.

The customer has the option of providing the needed verification by fax, mail, online or dropping the information off at one of the local offices.

Intake Process:

Apply online, print off application from website or call to request an application to be mailed to you. DHS offices accept applications from walk-in clients. Online applications may be filed by creating an account at www.tn.gov/humanservices. With a online account, application documents may be uploaded for processing.

Eligibility:

Applicants must be US citizens or legal aliens, reside in TN county in which they are applying, have access to cooking facilities (homeless shelters acceptable), and meet income guidelines based on federal poverty level. Income of entire household used to determine eligibility.
