Description:
Provides support coordination for persons receiving the HCBS Medicaid Waiver services from the TN Dept of Intellectual Disabilities (DIDD). Support coordination is provided by an Independent Support Coordinator (ISC) who develops, monitors and implements a Plan of Care to help persons choose paid services and access other supports to enhance their independence, productivity and integration into the community. The Union City office is the main administrative office for TNCSA.

Helps to identify, locate and assess all propsed and necessary services and informal community supports, including reimbursement for day care, respite care, specialized supplies, home and vehicle modifications and equipment.
