Diabetes Clinics & Patient Education by Referral
Patient Pharmacy Assistance Program
Individualized Christian Counseling by Trained Lay and/or Ordained Ministers

Hours:

Clinic Hours: Open on the 2nd, 3rd and 4th Tuesday of each month from 5:00 - 8:30pm, first come, first served. Limited to the first 25 patients. Doctors start seeing patients at 6:00pm.

Staff Hours: Monday, Tuesday, Wednesday from 10am-4:00pm. Call clinic in advance at 423-256-2408 to make sure someone is in the office.

Intake Process:

Applications can be picked up at the clinic on Monday through Wednesday from 10am-4pm. Call in advance to be sure someone is in office.

You must apply to become a patient at the free clinic. Eligibility applications are processed on the first Thursday of each month from 10am to 4pm.

Patients must be seen at our evening clinics before being scheduled for followup at our diabetes clinics or for other services.

Program Fees:

Office Visit: $5
Diabetes Test Strips: $5
Processing fee for medications obtained from pharmaceutical companies on behalf of patients: $5

Eligibility:

We serve patients who are UNINSURED and whose incomes fall within 185% of the federal poverty level.
