Hours vary depending on client and case manager. Services last anywhere between 3 and 4 months.

Eligibility

Individuals who are transitioning from a higher level of care or do not meet medical necessity criteria for a more intensive service. Individuals may also receive Case Management services in conjunction with Intensive Outpatient and/or Medication Management to coordinate care and engage client's external support systems.

Funded by DCS and Managed Care Organizations.

Description

Provides integrated treatment for co-occurring substance abuse and mental health issues to both youth (12-17) and adults 18 and older. Services are designed to prevent higher levels of care, help sustain long-term sobriety, good mental health and support clients remaining in their permanent homes and environment.

Hours

Hours vary per clients and program, but groups meet up to 4 times per week and up to 9 hours per week.

Eligibility

Adolescents and adults with alcohol and drug addiction, and in need of life enhancing recovery from co-occurring conditions. Adolescents and adults with significant mental health diagnosis in need of life enhancing skills to assist in coping with their diagnosis.

Funded through/by: Insurance, Self Pay, and DCS.

Intake Procedure

Call the Savannah Region office for more information and a brief screening. After a screening a counselor will schedule an evaluation, if the individual qualifies for IOP they will be contacted by a counselor as soon as possible.

Description

Provides in home counseling that focuses on building skills and support of the child and family to prevent placement outside the home and stabilize current placement.

Hours

Duration and hours of services depend on the needs of the client/family and is at the discretion of the referring worker.

Eligibility

Individuals must display the following risk factors: Primary diagnosis of a major mental illness, Evidence of a severe emotional disturbance, High risk of potential psychiatric hospitalization or other out of home therapeutic setting -Returning from an out of home therapeutic setting.

Description

Provides crisis treatment, individual and family counseling, communication skills, case management activities, coordination with other required services, and 24-hour emergency response. Services are intensive and time-limited.

Hours

Services are provided 3 times per week for a minimum of four weeks, depending upon the severity of the case, and may extend up to 16 weeks. Funding sources can increase in the number of sessions per week or extend the length of service if a greater medical necessity is determined.

Eligibility

Children and adolescents who demonstrate a clinical need that comes from a mental, substance abuse, behavioral, or emotional illness that puts them at risk for removal from the home.

Description

Provides a wide range of interventions tailored to individual child and family needs to children and adults that present with the following substance abuse and/or mental health issues: Depression, Alcohol and Drug Counseling, ADHD, Grief, Anger Management, Bipolar Disorder, Autism Spectrum Disorder, and Attachment Disorder.

Hours

Individual, family, and group therapy are conducted on a weekly basis (more or less frequently as deemed necessary. The duration of treatment varies depending on clinical needs of the client.

Children and Adults that present with substance abuse and/or mental health issues.

Provided to children and family members who are impacted by substance abuse. Provides an evidenced based treatment of alcohol and drug abuse and dependence. The service can be provided in the home or office setting and can be delivered on an individual, group, or family basis. Services provide intervention, support, and referral.
