Description:
Provides a variety of programs to assist adults 18 years old and over with mental illness and co-occurring disorders of mental illness and substance use. The programs at Park Center East include homeless outreach services and a psychiatric rehabilitation day program. The homeless outreach services consist of two components: Street Outreach and SOAR (SSI/SSDI outreach, access, and recovery). SOAR coordinators work to increase access to SSI/SSDI (supplemental security income/social security disability income) benefits for individuals by guiding them through the social security application process. The street outreach program identifies people living on the streets through outreach and engagement strategies; assists them in obtaining vital records; and offers case management, transportation and housing. The PSR (Psychiatric Rehabilitation Services) Day program is for people who have mental illness and substance use disorders. Offers individual coaching and support to adults in recovery. Offers a variety of groups which provide education and practice in coping skills including: co-occurring groups, art therapy, therapeutic drum circles, community integration, yoga classes, nutrition and wellness classes, and independent living skills (bus training, budgeting, etc.)

Eligibility
- Individuals (ages 18 and over) with a diagnosis of severe mental illness. - Must have TennCare - Not appropriate for individuals with intellectual disabilities who have an IQ less than 70.

Eligibility
The SOAR program is available to the following individuals: - Adults in Davidson County - Must currently be in mental health treatment for a severe mental illness that is expected to last 12 months (therapy, medication management) - Must be unhoused or having a housing crisis. - Cannot have an SSIS/SSDI case currently pending.
