Description

Medical Case Management (MCM) works on a complete continuum of care based upon a service plan to include but not limited to medical, nutrition, and other immediate needs. Street Works staff promotes, in every session, treatment adherence, literacy, healthier eating and social habits, job readiness or career development, family reunification and education, community engagement and HIV advocacy and spiritual growth. Medical Case Manager: provides comprehensive assessments to determine the needs and level of literacy, mobility, understanding and capacity of newly diagnosed individuals and, with them, develops, implements and maintains a Care Plan tailored to their individual goals in terms of HIV treatment, medical and mental health care; makes referrals to community based organizations that address participants specific needs; provides emotional and Social Support to reduce depression and isolation; and provides transportation when needed.

    Description

    Provides HIV/AIDS testing and counseling to individuals and couples in need. HIV counseling and testing (C&T) is an important part of Street Works continuum of HIV prevention and treatment services. C&T is one of the main times when a comprehensive individual risk assessment is taken, making it the best opportunity for accurate referrals to more intensive services. C&T is also one of the primary entry points into prevention and other services. C&T uses short, client-centered counseling that can be effective in increasing condom use and preventing sexually transmitted infections (STIs). Street Works uses a mouth swab (OraQuick) that tests cells from inside the cheek. With this method, results are available in 20 minutes, eliminating the need for a return visit for results.

      Description

      The Street Works Emergency Financial Assistance program allows client to apply to have some of their costs related to their housing covered through various grants, including the Housing Opportunities for People with AIDS (HOPWA), Ryan White Part A, and Broadway Cares. Clients can apply for move-in costs for permanent housing such as security deposits and first month’s rent and utility deposits. In addition, Street Works can sometimes provide assistance to clients who are struggling to make their rent, mortgage, or utility payments to avoid eviction or loss of service. The Housing Case Manager can also help Clients to resolve landlord disputes, secure outside financial assistance and counseling, develop independent living skills, and when necessary, find alternative affordable housing.

        Description

        Street Works provides services designed to link clients to both emergency shelter and permanent housing. Clients are given a full assessment to determine what housing and treatment options might be available to them based on their history and personal goals. The Housing Case Manager can then assist the Client in completing applications for various housing options–always working to find an income-based housing option first–including public housing, the Housing Choice Voucher Program, subsidized housing, substance abuse and mental health treatment and recovery centers, recuperative care, and private landlords. Clients are also given counseling on how to be more marketable to potential landlords, assistance with obtaining the important documents necessary for most housing applications, and referred to services which can help them improve their personal and financial health so they will be well-suited to live independently.
