Description:
Provides comprehensive, community based, in-home services which include health assessments, screenings, and parenting education provided by Registered Nurses and Licensed Social Workers. Length and intensity of services are flexible and based on a holistic assessment of the child, caregiver, and family needs. Services can begin prenatally through the baby turning two years old, with the goal of promoting healthy pregnancy, infancy, and early childhood development.

Mon - Fri 9:30 am - 4:30 pm

Intake Process:
* Receive referral from physician, caseworker, case manager, social worker, health department, or other care provider; someone with a professional relationship with the family.
* Go on website to complete online referral form at https://nursesfornewborns.org/tennessee/referrals/

Interpreter services provided for all languages.
Eligibility:
Open to residents of Cheatham, Davidson, Maury, Montgomery, Robertson, Rutherford, Sumner, or Williamson County. Referrals of residents in Wilson and Dickson Counties are considered case by case.
Must also be one of the following:
* Teen parent, 19 years of age or younger
* Baby who is medically fragile (e.g. born prematurely, spent time in neonatal intensive care unit, heart issues, etc.)
* Mother or expectant mother who has medical, mental health, or substance use concerns

Agency needs diapers, baby wipes, portable cribs, bottles, formula, baby food, clothing, toys, and blankets. Donated items may be new or gently used. Will also accept car seats that are brand new in the box or that have tags in place. * NOTE: Donations are to be used for women and infants, who are already in the program, only. Donations are not given to general public.
