Provides absentee ballot information. Requests for absentee ballots should be picked up and MAILED TO: Shelby County Election Commission, PO Box 342798, Bartlett, TN 38134-2798. FORM MUST BE DELIVERED BY USPS, FEDEX, UPS OR A COURIER SERVICE. DOWNLOAD FORM FROM WEBSITE OR CALL TO REQUEST A FORM TO BE MAILED TO YOU OVER THE PHONE. NOTE: DUE TO COVID-19, IN JULY 2020 CHANCELLOR (JUDGE) ELLEN HOBBS LYLE DECREED THAT ANY ELIGIBLE (CURRENTLY REGISTERED) VOTER MAY REQUEST AN ABSENTEE BALLOT AND THIS WAS AMENDED IN AUGUST 2020 TO ONLY INCLUDE MEDICALLY VULNERABLE PERSONS AND THEIR CARETAKERS, DUE TO COVID-19; SEE WEBSITE FOR SPECIFIC DETAILS ON ELIGIBILITY AND DOCUMENTS NEEDED. TO DO SO, YOU MUST SUBMIT A WRITTEN REQUEST FOR AN ABSENTEE BALLOT BY MAIL, FAX OR EMAIL TO YOUR LOCAL COUNTY ELECTION COMMISSION OFFICE BY THE SEVENTH DAY BEFORE THE ELECTION DAY. YOU CAN SUBMIT YOUR WRITTEN REQUEST BY MAIL, FAX OR EMAIL; IF YOU ARE EMAILING YOUR REQUEST, BE SURE THE ATTACHED DOCUMENT CONTAINS THE INFORMATION BELOW AND YOUR SCANNED SIGNATURE: 1. NAME OF THE REGISTERED VOTER; 2. ADDRESS OF THE VOTER; 3. VOTER'S SOCIAL SECURITY NUMBER; 4. VOTER'S DATE OF BIRTH; 5. ADDRESS TO MAIL THE BALLOT; 6. THE ELECTIONS IN WHICH THE VOTER WISHES TO PARTICIPATE; IF THE ELECTION INVOLVES A PRIMARY, THE POLITICAL PARTY IN WHICH THE VOTER WISHES TO PARTICIPATE; 7. REASON THE VOTER WISHES TO VOTE ABSENTEE (FOR EXAMPLE: YOU ARE MEDICALLY VULNERABLE, HAVE A DISABILITY OR ARE A CARETAKER OF A VULNERABLE PERSON); 8. VOTER'S SIGNATURE. NOTE: YOU MUST REQUEST A SEPARATE ABSENTEE BALLOT FOR EACH ELECTION!
A REQUEST THAT CONTAINS THIS INFORMATION WILL BE PROCESSED AND A BALLOT WILL BE MAILED TO THE VOTER.

NOTE: AS OF AUGUST 2020, THE COVID-19 OUTBREAK IS AN ACCEPTABLE REASON FOR PERSONS WITH MEDICAL ISSUES, DISABILITIES OR THOSE WHO ARE CARETAKERS TO REQUEST AN ABSENTEE BALLOT AS WELL. APPLICATION IS AVAILABLE AT WEBSITE. Shelby County voters; Absentee registrants may cast ballots by mail for being out of county, full-time student or spouse of full-time student, being hospitalized/sick/physically disabled or caretaker of same (Permanent absentee voter must have statement from doctor), juror in federal or state court, observing religious holiday, being candidate for office in the election, ages 65 and above. Registration open until 30 days before election. Register in person at Poplar Avenue address or at the Election Operations Center or by mail with postmark by close of registration. If registered by mail, you must vote in person the first time. Absentee voter must send written request with name/address as it appears on voter registration or registration number, election wanting to vote in, reason for requesting absentee ballot, address for mailing ballot,and signature. Send requests to Poplar address. Ballots sent if requested information is correct. Requests accepted no more than 90 days and no fewer than 7 days before election. Once received, ballot must be returned by 7:00 pm on election day
