Provides general medical care, pediatrics, pharmacies (located at Broad Avenue, Hickory Hill, Orange Mound and Third St Clinics), HIV care, prenatal care, birth control and family planning.
Hours:
Mon - Fri 8:00 am - 5:00 pm; Pharmacies closed each day from Noon - 1:00 pm for lunch; clinics open one Saturday per month; call clinic for dates/hours
Intake Process:
New patients call for appointment, beginning at 7:30 am. Current patients can call 2 days in advance to book appointment. Bring photo ID, insurance card, medicine and child's shot records to each visit. If self-pay, new patients need to bring proof of income and residence. Refill requests from your pharmacy should be faxed to the clinic where you have been treated.
Program Fees:
Fees Vary. Fees for users without insurance based on income and household size. Medical care fees range from $25.00-$85.00 based on income and household size. Prenatal care package, $1200.00, includes all prenatal visits, ultrasounds on-site and physician delivery fee, but not lab work. Lab work fees are discounted based on income and household size. For services requiring a lab, the fees include the cost of the lab and the discounted cost of the procedure. Accepts TennCare and most private insurance plans.
