Description:

Provides child health services, dental care for children on TennCare or uninsured children (children's dental care includes screenings, exams, sealants and referral to pediatric dental specialist if necessary) and adult dental care on emergency basis only (includes an exam and extraction of one tooth); developmental delays screenings, immunizations, primary care services for adults ages 19-64 (including exams/treatment and limited pharmacy), sexually-transmitted disease control, tuberculosis screening and treatment, nutrition education, WIC screenings, prenatal care, vital records (birth and death certificates recorded in Tipton County), environmental health inspections, family planning services including pregnancy tests and contraception and TennCare outreach.

Mon - Fri 8:00 am - 4:30 pm

Intake Process:

Call for appointment or walk in; State issued photo ID, medical documents; for WIC applicants, also need proof of residence and income, birth certificate, proof of citizenship

Program Fees:

Fees vary by service; sliding scale fee with a minimum charge of $5.00 on most services; No fee for WIC screening.
$15.00 all copies: birth and death certificates; only have birth certificates from 1949 - present day; prior to 1949 must obtain in Nashville

Languages:

Spanish, Interpreter Available for other languages

Eligibility:

Tipton County residents needing health care with no health insurance or persons needing vital records recorded in the county; to be eligible for primary or adult dental care must be aged 19-64 with no insurance; children must be on TennCare or with no insurance
