Description:
Provides cremation permits, burial transit permits, death certificates, birth certificates, post-disaster death certificates, and marriage and divorce records for Tennesseans. Accepts applications for birth certificates. Copies of death certificates and copies of birth certificates for persons born in TN in 1949 or after may be requested through the county health department. No birth certificates are available in TN for people born prior to 1914.

Birth Certificate Short Form - $15.00
Death Certificate - $15.00
Marriage Certificate - $15.00
Divorce Certificate - $15.00
Additional copies for any certificate is $15.00 per copy.
