Description:

Provides dental care for both children and adults, which includes exams, cleanings, sealings, extracts, limited periodontal treatment, restorations, dentures and partials, crowns,and root canals. TennCare and other insurance accepted or sliding-scale fee.

Hours:

Dental Clinic Hours: Mon - Fri 8:00 am - 8:00 pm; Tue & Thu 8:00 am - 7:00 pm; Fri 8:00 am - 5:00 pm (Pharmacy closed for lunch each day from 1:00 pm - 2:00 pm)

Intake Process:

Call for appointment, beginning at 7:30 am. Bring photo ID, insurance card, medicine and child's shot records (if for child) to each visit. If self-pay, new patients need to bring proof of income and proof of residence (acceptable proofs of income include a pay stub from the last 30 days, a bank statement, an income tax return from last year and DHS or unemployment information.) Pharmacies are closed for lunch each day 1:00 pm - 2:00 pm. Refill requests from your pharmacy should be faxed to the clinic where you have been treated.

Program Fees:

Fees for users without insurance based on ability to pay. Dental care discounts range from 60%-75% depending on income and household size. For services requiring a lab, the fees include the cost of the lab and the discounted cost of the procedure. Accepts TennCare and most private insurance plans; deductable payment is expected at time of visit.
