Description:
Provides child health services/developmental delays screenings, immunizations, sexually transmitted disease control (including confidential testing, treatment and prevention) tuberculosis control, nutrition education, WIC, pregnancy testing and presumptive eligibility screening for Medicaid for pregnant women, vital records (provides death certificates for Lake County residents and accepts applications for birth certificates for the entire state of TN), environmental health inspections, and TennCare outreach.

Program Fees:
Sliding Scale Fee, some services may be free; Copies of death certificates are issued upon request at a cost of $15.00/copy. Certified copies of birth certificates for persons born in TN for all years may be requested through the county health dept if available at a cost of $15.00/copy for persons born on or after 1949; and $15.00/copy for persons born between 1914-1949. (No birth certificates are available in TN prior to 1914.) Additional copies of the same person's record requested at the same time are $15.00/copy.
