Health Connect America is a company that provides behavioral health treatment and related services. The agency's network of caseworkers, therapists and clinicians are readily available to meet the needs of families in our community. Offers both in-home and outpatient services for at risk youth, families, and communities, with the steadfast goal of promoting reunification and/or permanency goals for all parties involved.

Mon - Fri 9:00 am - 5:00 pm

Program Fees:

Medicaid/TennCare, Straight Fee or Other Fee

Eligibility:

Eligible clients are children and adolescents who demonstrate a clinical necessity arising from a condition due to mental, substance abuse, behavioral, or emotional illness that results in sign significant functional impairments in major life activities and placing them at risk of removal form their home.
