Description:
Adult Day Services (ADS) is a licensed, social model day program that provides caregiver support and respite, socialization, and engaging activities and programming to adults 50 and over with physical and/or cognitive needs. This program assesses each participants's needs through completion of an assessment and development of an individualized plan of care. ADS staff complete daily monitoring of participants' physical and emotional condition and provide tailored group and individual programming to enhance the participants' functioning. ADS participants also receive nutritious meals.

Sliding scale with a limited number of subsidized slots for low-income seniors
Eligibility:
* Open to adults, ages 50 or older who are mentally or socially isolated and/or have a mental or physical disability
* Must be able to pay sliding fee scale or have an income within the funding guidelines
* Must have the ability to use the bathroom independently, feed self, and walk without assistance
* Must be able to participate in group activities
No-cost services and transportation are available for Davidson County customers.
