FiftyForward Martin provides a variety of engaging, fun, and educational activities for adults 50 and older, including but not limited to exercise classes, educational courses, arts and crafts, music, travel, and more. As an organization, FiftyForward provides access to numerous supportive care services for older adults in Davidson and Williamson County, TN.

Description
Care Management is a program specifically for seniors, ages 50 and older, who have a limited income and don't already have access to a care manager. Benefits and services include: - In-home visit to determine needs - Linkage, application, and advocacy with service providers - Monitoring to ensure that services are received in a timely manner - Temporary, limited in-home services (homemaker, meal prep, errand running, and escorted transportation) while on waiting lists for similar services

Description
Conservator program for individuals who can no longer make safe or appropriate decisions for themselves due to cognitive and/or physical disabilities. Benefits include: - A healthcare advocate for decision making - Assistance in managing income and assets - Help in maintaining an active lifestyle, family relationships, and independence - Regular in-home visits - Court oversight of all decisions made regarding sale of property and management of finances - 24/7 access to staff

Victory Over Crime is a program for older adults, who have been victimized by crime. Services include, but are not limited to case management, legal advocacy, in-home services, and counseling support.
