Mon - Fri 8:00 am - 5:00 pm office hours;
Visiting hours: 7:00 am - 9:00 pm;

Intake Process:
Walk-in or call (865) 992-5816, walk-in, or be referred by a healthcare provider, usually a hospital. Documents required:
State ID, Social Security Card, proof of income, birth certificate, medical records, drivers license. Every patient applying for Medicaid must have bank statement and life insurance information.
Program Fees:
Fees vary. Accepts Medicaid/TennCare, Medicare and Private Insurance, long term care policies in network. If a patient's insurance policy
is not already in contract with Willow Ridge, Willow Ridge can negotiate an agreement.
Eligibility:
Must have doctors orders for placement, and be safe for the population at Willow Ridge. No locked rooms/halls.
