Provides case management services, financial assistance (limited on only certain zip codes in Davidson County), food assistance (3 different programs), referral/linkage to other agencies and non-profits, diapers and baby formula and clothing and items for infants and small children, MDHA housing registration assistance, household items and cleaning supplies and hygiene items, SNAP enrollment assistance and budgeting assistance in Davidson County. At this time, financial literacy and personal financial counseling are no longer a requirement for persons seeking financial assistance. When meeting with clients, the Outreach Case Manager works with clients to create a budget. Clients in need of additional financial counseling are referred to UWGN!s Financial Empowerment Center.
Persons in need of counseling, individual and family, are referred to the main office (615-352-3087) in Donelson at 2806 McGavock Pike.
The agency's food pantry offers food boxes with food to cook, USDA and Commodities Senior Food Program for eligible Davidson County residents. Other social services (SNAP enrollment, budgeting help, referrals, clothing, diapers and baby formula for infants and small children, MDHA housing registration help and household items, cleaning supplies and hygiene items), are open to all in need in Davidson County. The financial assistance program offers very limited amounts of utility (electric, water and gas) and rent or mortgage assistance to residents of following zip codes/areas: 37143 (the Nashville-Davidson County side only); 37189; 37203 (northern portion only, near Charlotte Avenue); 37205; 37208; 37209; 37218; 37221; and 37228. Financial assistance for rent, mortgage, electricity, water, or gas help is zip code specific.

Intake Process:
