Provides Midtown Clinic which offers a variety of outpatient programs for adults with severe and persistent mental illness and serious emotional disturbances. Services include 1200 Peabody:
Outpatient Services ! psychiatry, individual therapy, case management/care coordination, supportive employment services
Recovery Services ! psychiatry, MAT, individual & group therapy, peer support
ACT ! Assertive Community Treatment ! Medication Management, Individual Therapy, Family Therapy, Group Therapy, IMR, Care Coordination, Peer Support
OnTrack TN ! first episode psychosis program ! Medication management, individual and group therapy, case management, certified peer and family support, Supported Education and Employment Services
Hours:
Mon, Tue & Thu 8:00 am - 4:30 pm; Wed 8:00 am - 5:30 pm; Fri 8:00 am - 3:30 pm
Intake Process:
Call for appointment or walk in (this office accepts walk ins)
Program Fees:
Accepts TennCare, Medicare/Medicare dual (with TennCare as secondary form of insurance) and offers a sliding scale fee based on federal poverty guidelines for persons without insurance
