Description:
Provides services to adults with intellectual disabilities through the State's Medicaid waiver program. Services include Residential Habilitation, Supported Living, Semi-Independent Living, Personal Assistance, Respite, and Community Participation. The goal of all services is to help the person supported with activities of daily living and community integration in its various forms. Also provides Early Intervention Services to children with developmental delays from birth up to age five (5); and provides Family Support which provides financial assistance to children and adults with severe life-long disabilities.

Children and adults with intellectual and developmental disabilities in Robertson, Sumner, and Trousdale County; Eligibility for
Medicaid waiver services ! must have a diagnosis of Intellectual Disability and reside in Robertson, Sumner or Trousdale County;
Early Intervention Service ! child must have a diagnosis that places him/her at risk for a developmental delay or who has been assessed to have a 25% developmental delay in two areas or a 40% delay in one area; Family Support ! person must have a severe life-long disability.

Description

Provides financial assistance to individuals with severe disabilities and their families. Assistance can be used for respite care, day care services, home modifications, equipment, supplies, personal assistance, transportation, homemaker services, health related needs, nursing and counseling.
