Description:

Provides professional home health care including skilled nursing, infusion therapy, physical therapy, occupational therapy, speech therapy, medicine management, fall prevention, low vision program, personal care assistance, wound care therapy, behavioral health nursing, medical social services and complimentary subscription to Lifeline for qualified patients. To receive Lifeline service the patient must have a land-line phone and if they lose their qualification they can still have Lifeline, at a reduced fee.
