The Greater Nashville Regional Council (GNRC) helps to promote, screen, and coordinate a variety of home and community-based services on behalf of state and federal funding programs. The programs focus on connecting applicants to agencies that provide a range of life-sustaining support services, including in-home respite. The TennCare CHOICES Program provides long-term care for clients who qualify for Medicaid and meet the level of care that could be provided in a nursing home. In-home respite care is one of many services provided through TennCare CHOICES. OPTIONS for Community Living is a state-funded program that, like CHOICES, offers in-home respite. OPTIONS is considered a last-resort program for those who do not qualify for CHOICES. GNRC facilitates the Family Caregiver Support Program, which funds in-home or personal care respite for family and informal caregivers. To qualify for CHOICES, you must be a Tennessee resident who is 65 years of age or older, or 21 years of age or older with a physical disability and need the level of care provided in a nursing home and qualify for Medicaid long-term care. To be eligible for OPTIONS, the applicant must be a Tennessee resident who has difficulty caring for themselves or their home. If you are determined eligible for CHOICES or other programs, the OPTIONS program cannot provide services. The Information and Assistance program (I&A) consists of a helpline designed to provide information on aging & disability issues and community resources. This program is provided through the Area Agency on Aging and Disability. Our friendly and knowledgeable staff are available to discuss and provide information to best meet the needs of the caller. All staff are trained and certified under the Alliance of Information and Referral Systems as a Certified Aging Specialist. Our database of community resources is maintained and updated regularly so that accurate information can be disseminated to the Greater Nashville community. Greater Nashville Regional Council's is designated as the Area Agency of Aging and Disability (AAAD) by the Tennessee Commission on Aging & Disability (TCAD). The Greater Nashville AAAD is one of nine AAADs in Tennessee and one of over 600 across the country. The AAADs are part of an Aging Network of federal (Administration on Aging), state (TCAD) and local Provider agencies. The primary objective of the AAAD is to enhance the lives of older adults and individuals with disabilities by offering a range of services and support. Congregate meals, provide seniors with meals and the chance to socialize and participate in program activities with other seniors. A wide range of facilities host congregate meals throughout Middle Tennessee, including senior citizens centers, community centers and churches. The Greater Nashville Regional Councial (GNRC) is the Area Agency on Aging and Disability (AAAD) and contracts congregate meal facilities on behalf of the Older Americans Act and Tennessee Commission on Aging and Disability. The Greater Nashville Regional Council (GNRC) facilitates the Public Guardianship for the Elderly Program and contracts local providers to provide legal assistance and Ombudsman services for the region’s older population. The Public Guardianship for the Elderly Program provides services for individuals assigned to the agency by the Circuit, Chancery and Probate Court system. The program provides services to those persons 60 years of age and older who due to physical or mental limitations, are unable to meet the essential requirements of their physical health or to manage essential aspects of their financial resources, and have no family member, friend, or corporation willing or able to act on their behalf. For seniors looking for legal services, GNRC contracts with the Legal Aid Society of Middle TN and The Cumberlands (Legal Aid). Legal advice and representation are provided by an attorney to people age 60 and over to secure and maintain public benefits, to resolve consumer problems and other situations requiring the intervention of an attorney. Those interested in this service can contact Legal Aid directly by calling 1-800-238-1443 or GNRC's Information and Assistance Hotline for a referral. GNRC contracts with Mid-Cumberland Human Resource Agency (MCHRA) to provide Long-Term Care Ombudsman services for the 13-county Greater Nashville region. Long-Term Care Ombudsmen are advocates for residents of nursing homes, homes for the aged, assisted-living facilities and similar adult care facilities. They provide information and work to resolve problems or complaints of individual residents. The Ombudsman can also assist prospective residents or family members to find a facility that best meets the needs of the prospective resident. Home-delivered Nutrition Services, commonly referred to as Meals on Wheels, provide meals and related nutrition to older individuals who are homebound due to illness, disability, or isolation. The goals of these programs are to help older individuals maintain their independence in their own home, prioritizing individuals at the greatest social and economic risk. At minimum, home-delivered meals are provided five (5) or more days a week. Home deliveries may include more than one meal for each day’s consumption if the participant can safely store the meals. These meals may consist of hot, cold, frozen, dried, canned, fresh foods and any supplemental goods for any additional meals. GNRC contracts with the Metropolitan Government of Nashville (Metro Social Services) and Mid-Cumberland Human Resource Agency (Mid-Cumberland HRA) to offer congregate meals throughout GNRC’s 13 county service area..

You must be a resident of Tennessee and must meet limitation requirements for activities of daily living (i.e. bathing, dressing, maintaining continence, etc.) and instrumental activities of daily living (i.e. transportation, meal preparation, housework, managing medication, etc.). The Options for Community Living Program is considered the resource of last resort. If you are determined eligible for CHOICES or other programs, the Options program cannot provide services.

The Greater Nashville Regional Council provides Medicare information, education, and counseling through the Tennessee State Health Insurance Assistance Program (TNSHIP). TNSHIP offers unbiased, objective information free of cost to Medicare beneficiaries and to the community at large. The goal of TNSHIP is to assess, inform, advocate, protect, educate, and empower Medicare beneficiaries. SHIP along with the AAAD work closely with the Senior Medicare Patrol (SMP) to help detect Medicare fraud. SMP helps Medicare and beneficiaries avoid, detect, and report fraudulent practices. TennCare CHOICES in Long-Term Care is TennCare's program for long-term care services. Long-term care includes help doing everyday activities that you may not be able to do for yourself as you grow older, or if you have a physical disability. These are things like bathing, dressing, and preparing meals. Long-term care can be provided in a nursing home. Long-term care can also be provided at home, in the community, or in residential places like Assisted Living Facilities. Services to help a person stay at home or in the community are called Home and Community-Based Services (HCBS). The care you receive in CHOICES depends on your needs. CHOICES Home and Community Based Services (HCBS) may cover personal care visits, attendant care, homemaker services, home delivered meals, personal emergency response systems, adult day care, in home respite care, inpatient respite care, assistive technology, minor home modifications, pest control, community-based residential alternative and nursing home care.

The Greater Nashville Regional Council provides funding to senior centers across Middle Tennessee to support programs and services for older adults. Each GNRC contracted senior center offers telephone reassurance, providing comfort and connection while preventing loneliness for older adults who may otherwise be at risk for social isolation. Older adults looking to enroll in telephone reassurance services can call the GNRC information and assistance hotline to find a senior center near them.

Volunteers are an important part of the work the Greater Nashville Regional Council (GNRC) does in the communities across the region. GNRC offers a variety of opportunities for those willing to share their time with us, including: • Volunteer Medicare Counseling • Friendly Visitors for Guardianship Clients • Letter writing campaign to seniors and disabled individuals in nursing homes. • Information and Assistance hotline volunteers • Communications and Administrative volunteers Background screenings, initial and annual training may be required based on the volunteer opportunity and are provided for free by GNRC.
