Description:
Centerstone provides Supported Housing through contractual agreements with three managed care companies; Bluecare, Amerigroup and United Healthcare. Supported Housing is a program of care and is not considered permanent housing. Supported Housing program provides psychosocial rehabilitation services in licensed group home settings with 24/7 awake staffing. This service is available to adults 18 or older with a history of mental illness who are transitioning from either a psychiatric hospitalization, or an unstable living situation with a recent history of a psychiatric hospitalization within at least 3 months prior to the referral. The individual must be able to make gains in supported housing, so that step-down to a less restrictive setting can occur. Precertification approval by the assigned managed care company is required prior to admission, as the referral must meet guidelines for supported housing.

Provides specialized homes with additional 24/7 staffing that is available for those needing more hands-on care due to unique medical and/or mental health needs, including residents who require a mobility device or those who experience vision or hearing impairment.

Provides supervised and supportive housing for individuals in transition from psychiatric hospitalization or unstable living situations. Services include daily social and life skills training, coordinated whole-person health care, social outings, group activities, and more. There is a 24/7 staff on call to support residents.
