Description
Health Home offers a complete approach to care. Clients are assigned a care coordinator who reviews any medical conditions you may have â from depression to asthma to heart disease to arthritis â and how you are managing and treating them. The Health Home team will: work with you one-on-one provide you with resources to improve your quality of life create a care plan based on feedback from your medical and mental health care providers link you to resources to answer any questions help you schedule and arrange transportation to your appointments Your health home provider is available to listen and ensure you're receiving the appropriate follow-up care with your doctors and other health care professionals.
